The skin is opened on the back of the wrist 

The extensor retinaculum is raised. note the markings made with a sterile pen to show the incision lines

The retinaculum has been raised shoiwing the finger straigtening (extensor0 tendons underneath

The posterior interosseous nerve is displaced. It is divided to reduce wrist pain. (The anterior interosseous nerve is also divided)

The capsule over the distal radioulna joint (DRUJ) is divided to show the DRUJ beneath

The ulna head is removed
